Patient Privacy Authorization Form: This form needs to be signed by the patient and/or guardian in order for Lilly to have access to the patient's protected health information. This access is required so that Lilly may provide reimbursement services or injection training on the HumatroPen injection device. (Also available in Spanish)

Reconstitution (Mixing) System for the Humatrope Pen Injection Device

As part of Lilly's commitment to improving our products and services, the Humatrope reconstitution system has been designed with you in mind. The system includes a hidden needle and end cap so that it may be sealed for disposal. The reconstitution process involves just three easy steps:

  1. Push the Cartridge into the syringe.
  2. Press the plunger to release the diluent.
  3. Mix the Humatrope by inverting the cartridge.

The reconstitution system is intended for use with the HumatroPen injection device. The flexibility of dosing is maintained with a choice of 3 cartridges: 6 mg, 12 mg and 24 mg.
